I am not currently billing the following insurance companies. If you have out-of-network benefits I would be happy to provide you with a detailed receipt for your treatments as many plans will reimburse you.

**Please note that my being in network with your insurance company is not a guarantee of coverage. Insurance plans vary and not all plans include acupuncture coverage for all diagnoses. Please confirm with your insurance company that you have acupuncture coverage for your particular diagnosis before making an appointment. Should your insurance company deny the claim for your treatment, you will be responsible for the total.
** The Microsoft Premera plan does not include acupuncture coverage.
** Medicare does not cover acupuncture, and if you have Medicare your secondary insurance will not cover acupuncture either.
Fees:
Should you not have insurance coverage, or choose not to bill through your insurance, I have a payment at the time of service discount available which is as follows:
Acupuncture with herbal consult first visit: $120.
Acupuncture with herbal consult return visit: $85.
Seniors (over 65): $70
